All players in PentaSport MUST sign the waiver in order to play in the PentaSport Season. Teams with players not having signed the waiver will forfeit their games until all players on the roster have signed the waiver
Players age 18 or under as of March 26, 2024 must download the form at the following link and have their parent/guardian sign before uploading to the submitting their waiver on the online form.
